Services available to our patients:
- Care regardless of ability to pay, and without regard to age, race, creed, color, religion, sex, national origin, or handicap.
- Physicians and nurses available 24 hours a day, 7 days a week.
- Dedicated on-call nurses with prompt response will be at your service 24/7.
- Social Workers will work with you assisting with any insurance issues and will help connect you to any community resources available.
- Chaplains are there to provide you spiritual support or just companionship when needed, or to serve as a liaison to their faith community.
- Volunteers will be there to help with any need the family has, such as running errands, light housekeeping and relief time, etc.
- Care in the home for personal care by home health aides including housekeeping assistance.
- Nurses will visit weekly to assess a patient’s physical status and address any medicine needs for the patient or family.
- Hospice will pay for basic medical equipment needed in the home such as oxygen, hospital bed, and wheelchairs.
- Hospice will supply and pay for medical supplies related to hospice illness, such as: briefs, wound supplies, etc.
- Hospice will pay for medications related to the Hospice diagnosis.
- Relief care for up to 5 days for the patient to allow rest for the caregivers.
- Inpatient hospice care for acute symptom treatment when needed.
- Pharmacy services are available 24 hours 7 days a week.
- Specialty consultations for hospice related problems such as, physical/occupational/ speech and dietary therapy. These services are provided for symptoms related to the terminal illness upon physician order, through arrangement with contracted agencies or individuals.
- Any patient with limited English speaking ability or a medical handicap will be provided assistance so that the same quality of service is received by all patients.
- Bereavement care and grief support for the family 12 months following the loss of a loved one.
- Counseling is available to all patients’ and their families.
